Goals

  1. Enhance access to safe, high-quality, and person/family-centered healthcare in rural areas, ensuring that all community members can receive the care they need in a timely and effective manner.
    • Engage Clinicians: Actively involve clinicians in the development and implementation of evidence-based clinical care processes to reduce clinical variation. This can include regular workshops, feedback sessions, and the establishment of clinical practice guidelines tailored to rural health contexts. 
    • Population Health Management: Explore and implement shared population health management strategies that harness data analytics to identify at-risk populations and tailor services accordingly.  
  1. Broaden healthcare service offerings while optimizing operational efficiency to meet the diverse needs of rural populations effectively.
    • Service Availability Assessment: Conduct a thorough assessment of current healthcare service availability, including specialists, imaging, and laboratory services. Use this data to identify gaps and areas of expansion. 
    • Shared Resources: Determine opportunities for shared clinical and administrative resources among healthcare facilities to enhance service delivery.  
  1. Strengthen community health initiatives to minimize health disparities in rural populations, ensuring equitable access to care.
    • System of Care Development: Develop a robust system of care that engages community-based organizations to create a holistic support network for health and wellness. 
    • Provider Collaboration: Convene healthcare providers across the continuum of care to enhance coordination and reduce service duplication. Establish communication protocols and referral systems to streamline patient pathways.  
  1. Enhance the capacity of rural health organizations to thrive under value-based payment models, ensuring financial sustainability and improved patient outcomes.
    • Resource Pooling: Pool resources among healthcare providers to aggregate covered lives, enabling more effective financial risk management and negotiating power in value-based contracts. 
    • Contract Assessment: Systematically evaluate proposed value-based payment contracts to ensure alignment with organizational goals and the capacity to meet performance metrics.  
    • Financial Risk-Sharing Models: Develop innovative financial risk-sharing models that allow for collaborative contracting opportunities among Nebraska HVN members.  
  1. Achieve Financial Sustainability for Nebraska HVN and its member organizations.
    • Membership Contribution: Implement a minimal membership contribution model from each member organization to support operational costs. Communicate the value of membership clearly to ensure buy-in. 
    • Service Offering Expansion: Expand Nebraska HVN service offerings to members, focusing on those that deliver significant value.
    • Financial Sustainability Plan: Develop and execute a comprehensive financial sustainability plan that includes revenue diversification strategies, cost containment measures, and ongoing financial assessment to adapt to changing circumstances.

Status of Rural Healthcare

Challenges of Rural Healthcare​
In today’s evolving healthcare landscape, many of the most pressing challenges have led hospitals and physician groups to merge into larger health systems. These challenges include investing in advancements in diagnosis and treatment, controlling care costs, enhancing patient experiences, and fostering collaboration among clinicians. The fundamental reason behind this consolidation is scale. From enlarging patient populations and expanding physician networks to attracting top medical professionals – scale is often the answer.

For rural hospitals and clinics, achieving this scale can be an enormous undertaking. While merging with a larger health system is a viable option for some, many communities prefer to retain their local hospital’s undivided attention to cater to local needs.

Charting a Sustainable Path
The Nebraska High Value Network was conceived in recognition of these challenges, founded on the principle that rural hospitals and clinics can unite to address these issues and bolster their resilience while maintaining their independence.

Through our network, rural hospitals and clinics join forces on clinical and operational fronts to amplify accessibility, affordability, and quality of care in Nebraska’s communities. We are a beacon for more than 400,000 persons served, ensuring they receive nothing but the best care.

The Nebraska Clinical Integration Network (CIN) was established to further enhance our collaborative efforts. This empowers our member hospitals to offer continuous care to patients across the network, from surgery and ophthalmology to mental health and obstetrics. The CIN’s Clinical Integration Committee, comprising a practitioner from each member hospital, supervises all clinical and quality initiatives.

Looking to the Future​
Our exploration into collective solutions in areas like pharmacy, telehealth, IT, clinical staffing, and more is ongoing. Leading these ventures is our Business Integration Committee, a group of selected operations leaders from our member hospitals. This committee oversees our shared service offerings and serves as a collaborative platform for these leaders to address everyday business challenges and share resolutions.
